Camp and Shuttle Waiver *

- I give permission for my child to ride the Lions Legacy shuttle on July 6, 2025, to and from the Gabe Jeudy Football Camp. - I understand that Lions Legacy is not responsible for injuries or accidents at the camp or during transportation. - I release Lions Legacy and its volunteers from any liability for lost items, travel issues, or incidents related to this event. - I will ensure my child arrives at Lockhart High School on time (7:30 AM) and is picked up after the trip concludes. ☐ I agree that my child will show respectful behavior while participating.

Media & Activity Waiver:

By checking below, I give permission for Lions Legacy Athletic Academy (LLAA) to: Include my child in group photos and video from camp and transportation activities for promotional and highlight purposes (no personal info will be shared publicly). Act as a temporary guardian for camp check-in/out, emergency needs, and form signings if I, the parent/guardian, am not present at the event or on-site. Provide supervision and transportation support to and from camp under the care of LLAA staff or approved chaperones.
